Cancer is rapidly becoming the number one cause of death in the human population. It is not a single disease, but is actually 's of diseases arising from different tissues throughout the body and bearing characteristics unique to each individual. The most common question I hear with regard to cancer is, "do you think there will ever be a cure for cancer?
The answer is not so simple With cancer representing hundreds, if not thousands of diseases, finding a "cure" is not an easy task. One of the tenets of cancer, and a large part of what it makes it so difficult to treat in many cases, is its ability to adapt. Just like all living systems, when confronted with a strong environmental challenge, cancer evolves, it mutates, and it takes advantage of any natural system that is available in order to survive.
This is why so many modern treatments fail. What often happens is that because cancer cell populations are not homogenous, that is, there are cells bearing different genetics within a given tumor the oncological therapy destroys the cell population that is susceptible to its action, but some other population s within the tumor survive and grow - unaffected by the intervention.
This new population is resistant to the original therapy, and finding a way to target the new population can be very difficult. Plants and plant compounds are extremely valuable in this context because thet do not target a single pathway, or receptor - rather, they affect many pathways, and modulate activity.
These two methods are, however, invaluable as a confirming diagnosis based on interrogation and observation. The diagnostic pattern that would encompass all of the above is: Asthma caused by cold dampness obstructing of the lungs with spleen qi deficiency and kidney yang deficiency with liver qi stagnation. What of the Treatment Principle? While this may seem to be purely theoretical since the Treatment Principle just describes the strategy of treatment based an assessment of the patient and consequent diagnostic pattern previously given.
A “New” Strategy
Some may think formulating the treatment principle is obvious and therefore redundant. Our Treatment Principle outlines priorities of what needs to be done first or even by itself before the primary complaints associated with the asthmatic symptoms are treated. So in that case, the Treatment Principle would read:. Regulate Qi with herbs, diet, lifestyle adjustments and secondarily warm and dry Lung and Spleen dampness and tonify Kidney yang.
Based on the original diagnosis the treatment principle for this patient is congruent as follows: Warm and dry Lungs, dispel and transform phlegm, tonify Spleen and Kidney yang and regulate Liver Qi. Treatment Finally from all of the above we arrive at our herbal treatment by finding a representative formula. This could be any number of formulas one is most inclined to use but lets say it will be modified Citrus and Pinellia Er Chen Tang because of wise use of our reference materials. This dries and clears dampness and phlegm this type of dampness is not urinary dampness and Citrus and Pinellia can be used to prevent and treat some types of asthma.
However if there Wind symptoms that change associated with asthma, one might include an antispasmodic like lobelia inflata from Western herbal traditions or a low dose of ma huang ephedra which works in a completely different way to treat asthma it is like herbal adrenaline especially during an acute phase. We also see that coldness associated with yang deficiency is an issue in which case we would find a Kidney yang substance. Gecko lizard is specific Kidney yang substance that helps the Kidneys grasp the Qi of the Lungs like the Gecko grasps the ceilings and walls or branches of trees.
Generally, whenever possible it is good to add two herbs for every associated symptom you are try to treat. The reason is simply that two will have a positive synergistic effect with each other and affect a wider range of the condition. Its very useful for clinical practice. One may not find all the two herb combinations and in this case I might choose gecko and cinnamon twig because cinnamon twig also relieves external cold wind and joint pains and its warming action complements the use of gecko. For deeper low back pains we might add or substitute cinnamon bark as well.
Finally for greater kidney yang warming cinnamon with prepared aconite can be combined but for many aconite is too heating. Our studies should point to the use of xiao yao wan or Bupleurum and Peony Combination. This can be given as a separate formula or following dui yao principles, you may simply add bupleurum and white peony to the formula. The benefit of this systematic approach are many not least of which is that if we know our herbs well and we have reasonable trust in our diagnosis.
If any part of that logical process was in error, we have a good sense of what to change and alter. Usually if our diagnosis and treatment is precise, we can expect a positive result even of a chronic problem within a week or so. Channel and Connecting Vessel pattern discrimination based on an understanding of the acupuncture channels which is especially relevant to acupuncture therapy but an herb student should make it a point to know these because they are commonly referenced in formulas and clinical diagnosis. Next one should evolve a number of representative Traditional Chinese Formulas and then consider how the desired therapeutic effects can be accomplished to a greater or lesser extent with a Western or Ayurvedic protocol or simply by adding elements along with the TCM treatments and formulas.
This can be confusing but ultimately is what frees the practitioner to integrate healing herbs and substances from around the world. Most of the representative formulas are presented in the third part of the herb course and the second book of CTHM. Believe it or not through in depth mastery and understanding of a few formulas advanced herbalists often time will rely on a relatively small number of formulas say 30 or so with an understanding of how to vary them according to individual patterns.

We presented a study protocol of developing Chinese medicine clinical practice guidelines for three common diseases in Hong Kong, including insomnia, chronic gastritis, and cerebral infarction. This research project will be conducted in three phases.

First phase is the preparation stage which consists of the establishment of steering committee and panel. Second phase involves 6 steps, which are searching and identifying evidence, text mining process, Delphi survey, synthesizing of data, consensus conference, and drafting guidelines. In this phase, text mining technique, evidence-based method, and formal consensus method are combined to get consolidated supporting data as the source of CM CPGs.
The final phase comprised external reviews, dissemination, and updating. The outputs from this project will provide three CM CPGs for insomnia, chronic gastritis, and cerebral infarction for Hong Kong local use. They can improve health outcomes, the quality of clinical decisions, efficiency, and optimistic value for money [ 2 ]. Several major medical organizations, including the American Medical Association AMA and the Institute of Medicine IOM , have carefully formulated methodology for developing scientifically sound guidelines.
In Hong Kong, Hong Kong Academy of Medicine also developed local CPGs on the basis of the consensus of experts and the guidelines formulated by the major medical organizations worldwide mentioned above [ 3 ]. Chinese medicine CM is a traditional medical system that originated in China on the basis of clinical experience and theory, and it has, to a varying degree, been disseminated to and used in many countries. The increased use of CM has attracted public concern over the issue of its secure application. In the past few years, problems concerning the side effects induced by the incorrect use of CM have arisen e.
Most of the clinical practices of CM practitioners are still based on the individual experiences and lack the strong evidence to support the treatment scheme. The differences between CPGs in Hong Kong and mainland China would be the evidences involved or bilingual published language. In this study protocol we aim to develop CM CPGs in three areas which are of high disease burden and the evidence of effective practice is available in Hong Kong. The three areas include insomnia, cerebral infarction, and chronic gastritis. The methods will follow the general CPG development process [ 9 — 17 ] with modifications for achieving solid data.
The research program will be undertaken in 3 phases, including preparation phase, development phase, and finalization phase Figure 1. A panel is established as technical assistant team to prepare related guideline documents, review the literatures, conduct Delphi survey, organize the consensus conference, and draft and edit the CM CPGs. The aim of this step is to identify the best available evidence to address the related topics in three CM CPGs.
A systematic search of clinical research regarding CM treatments for insomnia, cerebral infarction, and chronic gastritis will be conducted. All relevant published studies in Chinese or English will be identified by searching the following databases: The following search terms will be used: Two reviewers will independently search the journal and select potentially relevant articles after screening the titles and abstracts.
In case of uncertain eligibility, the full text will be screened.

Eligibility criteria have been set as follows. Once the articles are selected as potential sources of evidence, the methodology used in each study will be assessed to ensure its validity. Two researchers will review the literature independently. For assessing the methodological quality of different types of study, we will use different tools as follows: Considering the characteristics of CM research, we utilize the well-accepted evidence grading systems for CM [ 21 ] established by Liu.
And the evidence body has been successfully used in the development of first evidence-based CM CPGs [ 8 ]. Steering committee will review integrated contents of existing CM CPGs including scope, target patients, and CM interventions to evaluate the possibility of local adaptation. The purpose of text mining process is to provide more information and data for developing CM CPGs, especially in those areas lacking high quality evidences, like classification of CM patterns. High frequency CM patterns and related CM treatments including Chinese herb treatments, acupuncture, and moxibustion will be obtained from the literature [ 23 ].
High frequency CM patterns and related CM treatments reported in clinical studies imply that they are well known by practitioners to some extent. We will use a data slicing algorithm based on the calculation of frequency, which is detailed in our previous research [ 24 ], to filter high frequency CM pattern and related CM treatments. To avoid repetition of clinical study and ensure efficiency, we will choose only one database as data source.
